Jeeves.co.in - Filing insurance claim against broken smartphone screen

I purchased a smartphone from flipkart along with which I purchased an insurance from jeeves covering accidental damage to the phone upto * year... Now when I filed for the insurance claim, i was directed to a webpage to upload some documents showing mobile damage along with some id proof....but there was no option on this webpage to submit these documents.... Their helpline number is always busy and flipkart too doesn't respond to any calls... Please help!
