Dear sir/madam
I opted for the toothsi treatment plan and after consultation with my assigned dentist I had given my tooth samples and paid an initial ammount of ***** as the token payment. Before token payment and my treatment commencement I had been informed that a nearby dentist clinic will be arranged to me for the basic clinical treatment but later I had been informed that no nearby centre is functional here in my location (Bokaro, Jharkhand) and I have to visit a centre in Kolkata or Patna. Both of the suggested centres are more than *** kms far from my residing place , and it's really very difficult for me to visit there as I have a toddler to look after.
Henceforth as it's not possible for me to continue the treatment kindly refund the paid token amount.
I had already been informed regarding my reasons for the discontinuation of treatment. But till date I haven't received any call for the leaving and refund processes.
Please do the needful for the refund at the earliest otherwise I will no other option instead of going to court or consumer forum .
