Hi there,
On dated **th August, I have requested to put my Dish TV account on temporarily deactivation from **th August onwards till my further instructions. I have emailed you from my registered email id and gave you written consent.
Earlier in the month of April ****, we mutually agreed that you shall not force me to talk or communicate over a phone call and you agreed upon. Please check your audio recording of my communication with your supervisor.
Now, I do hereby asking you to publish my active balance as of **th August and my account status.

Many thanks for your proactive approach and prompt action. This has accelerated and helped in resolving my issue.
