Subject: Repeated Wrong Rejection of Form-**C Claim
Respected Sir/Madam,
I, Kuldeep Choudhary (UAN: ************), would like to bring to your kind attention that my claim for Form-**C (EPS Withdrawal Benefit) has been repeatedly rejected on incorrect grounds.
- I had submitted my Form-**C claim online.
- My claim was rejected with the remark: “Already settled on ** Jan **** for Rs.****.”
- However, my EPF passbook (updated till **/**/****) still shows an EPS Balance of Rs.*,***.
- If this amount had been settled in ****, it would have been reflected in the passbook as a withdrawal entry and the balance would have been zero.
This clearly shows that there is a **system mismatch / incorrect update**, and I have not yet received the Pension Contribution (EPS) amount.
Therefore, I request you to kindly:
*. Recheck my Form-**C claim and process it correctly.
*. Release the pending EPS Balance of Rs.*,*** as shown in my passbook.
*. Take necessary corrective measures to avoid such errors in the future.
