Max Hospital Saket - Wrong Documents placed in my file

My patient Muzamil with Max ID SKIMS.****** was discharged on *-*-**** after PCD drainage was done. Very unfortunate to see in his file documents of some other patient that would have adverse effects on my patient if medicine of other patients were purchased and administered to my patient. I have came to this hospital for treatment of my patient because of the trust i gained in **** when my nephew was treated here with dedication from all staff members from top to bottom. But it was my mistake, today I observed lot of loops and breaches resulting a degraded image of this hospital.
