I Mr, Prashant working as a centre government employee and my salary account is in surat station road branch has been debited fraudently by Rs *****/- through various transactions on dated **/**/**** ( however transactions detail is showing in statement is **/**/**** ) at the time around ** : ** hrs with my family AT my home, I received sms on my mobile for ATM withdrawal as well as Card to Card transfer unauthorised transaction from my account. All transactions were made without any OTP or standing instructions by me or my family. Same time the ATM card was and is with my custody only and I never shared it with anyone else ever and I was at Surat at my Home. Immediately I called customer care number given in the sms from SBI and complaint against the fraud above mentioned unauthorised transactions and block my ATM card. I received complaint numbers immediate on my phone via sms. Next day I file complaint to Police station Surat and next I put every detail to the branch manager station road Surat. With the kind help of Branch manager and co operative staff same day it was forwarder to higher authority. Now sir, I request you to look into the matter and refund my debited amount total rs *****.**.
Sir, all the necessity, SOP forms with details copies and police complaint copy sent in prescribed forms duly signed by me on dated **/**/**** through SBI station road Surat branch through proper channel. The acknowledgement copy attached herewith for your kind info only.
Kindly take action against the fraud unauthorised transactions and revert back my money as complaint is already filed. Kindly provide CCTV footage of concern ATMs of Gaya ( Bihar ) when the transaction sms received ( sms screenshots attached for information)
