AFTER SO MANY CALLS & REMINDERS I HAVE NOT YET RECEVIED MY ORIGINAL POLICY AND PREMIUM PAID RECEIEPT AS BOTH ARE MUST FOR REIMBURSEMENTS FROM MY EMPLOYER. PLEASE ISSUE THE ON OR BEFORE **/*/** (LAST DAY OF SUBMISSION).
