Sir,
I purchased a health insurance policy for self and family members through online from Bajaj Allianz in the financial year ****-****. The date of commencement of policy was **.**.**** to **.**.**** (yearly basis). Further, I have renewed my policy for the next financial year ****-**** through online and the policy was issued from **.**.****-**.**.****.
Recently, my wife Smt Babita Sharma was admitted to BM Shah Hospital, Supela, Bhilai due to illness on **.**.**** at **.** am (Midnight). Under her medical treatment, the management of M/s B M Shah Hospital has sent approval for the medical treatment covered under health insurance on **.**.**** for mediclaim through the mail but the mediclaim approval was rejected from the health insurance company which is confirmed by the hospital. During the enquiry, I came to know that the policy is inactive and further the medical treatment is not assigned/covered under health insurance. As the treatment of my wife is not covered under health insurance for which I paid the total amount of Rs *****/- (Rs Sixteen Thousand One Hundred and Eighty Only) to the hospital. Further, I approach to the branch office in supela Bhilai but failed in getting the response that the policy card is blocked and they are unable to open it.
Further I want to clarify that why the policy is inactive or the account is blocked even I availed the policy amount of Rs *****/- on yearly basis with the sum insured of Rs **,**,*** (Rs ten lakhs only) though I am paying the premium of Rs ****/- (Rs One thousand One Hundred and Eight Only) on monthly basis for Loan no. – ***REP******** which is an auto-debit from UCO bank for which I have the transaction statement of the bank.
