Dr Harshit Acharya - Bill is not provided by the doctor

I visited Dr Harshit Acharya of Harshit Acharya Arogya Kendra address **/** harshid chamber opp epfo bldg near income tax ashram road Ahmedabad. He provided me with the bill of neither consultancy nor medicine. He does so to other patients as well. He does not provide any kind of bill to any patient. I want a proper bill from him so that I can get a medical claim from my company. Doctor Harshit acharya is strictly against paying the bill and giving even the medical prescription. Dr Harshit acharya mobile number is ********** and he has many years of experience (** years as he says) and he does not provide a bill to any patients means he does not give the proper tax to govt from many years. I have the only photo of his medicine which I hereby attach. Please do needful as the case is not only important for me personally but for the government as well as he stealing government tax for patient consultancy and medicine.
