Independent tv set up box - Most of the channels are not working

We have installed an independent tv set up box on *th March **** but after * to * days the most number of channels stop on the screen. I complaint most of the times to customer care service and to the operator but it doesn't solve. Hungama, Nick, Discovery and many more channels are not working. My set up box no. is ***************.

Bought independent tv in the month of April **. But it is not working at all from **th July **.so kindly take necessary action against it to refund the amount. Thanks
