Ford hospital patna Bihar - medical negligence of Ford Hospital Patna Bihar caused patient death

My Sister Mrs. Shwet Nisha Rani(patient Regn.ID IPD-****) was admitted at Ford hospital Patna(adderss New Bypass NH-** , Khemnichak , PO - New Jaganpura, PS - Ramkrishna Nagar Patna – ******,Bihar) on **.**.**** at **.** AM with pregnancy related complication of her twins. My sister was blessed with twins child (one baby girl and one baby boy) on **.**.**** at around *.** AM. Although the child was delivered with normal procedure (Without C Section at hospital) but placenta did not came out automatically within ** minutes. The Gynecologist Mrs. Anita Singh informed us that a minor procedure need to be performed to remove placenta and procedure for same was done after some time.
Despite the procedure, the part of placenta Size ***** mm was left in lower uterine segment. No Sonograply was done after procedure to ascertain the complete removal of placenta. The same was done with ** hours delay at **.** PM on **.**.**.Till then sever urine infection had already been spread.
Due to complication of placenta accrete (residual of part of placenta), patient suffered severe blood loss after delivery of twins. Hemoglobin count of patient decreased from **.* (at **.** AM on **.**.**) to *.* (at **.** AM on **.**.**) within * hours. Although patient was in ICU and sever blood loss of almost * to * unites occurred, due to negligence of hospital and ICU doctors the blood loss got unnoticed. The blood loss was noticed only after blood test report of **th morning at **AM approx. Both the blood test Blood test report has been attached for reference. Due to delayed diagnosis, blood transfusion was started only after ** AM on **.**.** with the delay of * hours.
Due to huge blood loss after delivery and due to delayed blood transfusion, Blood pressure of patient got extremely low and patient went on ventilator on **.**.** at approx. **.** PM after ** hours of delivery.
To the height of insensitivity and shamelessness , The team of doctors Mrs. Anita (Gynac), Dr. Dharmandra (ICU In charge) Dr. Gayan (Nephrologist) lead by Dr. Junjhunwala (Medical superintendent) got involved to cover up whole incident and tried to mislead the family of patient by diverting their attention towards other medical complication like kidney failure, liver failure, multi organ failure. Although in patient these medical issues were never pre historic except mild jaundice (Bilrubine level *.**, SGPT ***, SGOT ***). Some of these effect may have induced and reflected after huge blood loss and due to septic shock of placenta accrete when patient went on ventilator.
It is to be notice that hospital own Liver function test and Kidney function test report performed on **.**.** at *.** AM before surgery contradict hospital statements regarding kidney and liver failure issue. As Kidney function test reports was normal and liver function test also reflect only mild jaundice which is very normal during pregnancy. The above test Reports cannot be basis to conclude as liver or kidney failure by any certified medical professional. Even blood test reports of **.**.** at **.**AM is also within acceptable limit. The test reports has been attached for reference.
Even when patient was on ventilator and his condition (Blood Pressure***/**, O* **, pulse ***) came to normal after medication on **.**.**, no effort was made to remove the left out placenta to stop further infection. When we discussed the solography reports and about left out placenta it was told by gynac Mrs. Anita singh that left out part of placenta is not a serious issue and same will come out automatically or will be removed by surgery. That was never happened and caused severe infection after two days. The whole narrative was prepared to cover up medical negligence and to hide the failure of hospital. In this way patient was left out to die tragically with slow and very painful death.
Ultimately patient suffered septic shock followed by severe infection, extremely low BP, cardiac arrest and died on **.**.**** due to complete negligence and complacency of hospital and its team of doctors.
