Subject- Medical Negligence Caused to Patient Mr. Rajesh Mehta (PT ID KH**********)

I, the Undersigned Patient Mr. Rajesh Mehta **/M PT ID KH********** was admitted in your Hospital on **/**/**** till **/**/**** having diagnosis Right Sided Renal Calculus with Non Functioning Left Kidney. I underwent surgical intervention in the form of Right PCNL with DJ Stenting on **/**/**** performed by Dr. Bejoy Abraham. I was catheterized with Foleys catherter and which was planned for removal before my discharge from the hospital. On **/**/**** one day before my discharge my Foleys catheter was to be removed as per information given to me by my treating surgeon Dr. Bejoy Abraham . But on **/**/**** when my Foleys catheter was being removed it was found that my Foleys catheter was got stuck up inside the urinary bladder and it was not getting easily removed by on duty staff. Hence my treating surgeon was informed about this incidence and he came and he also tried to remove the catheter but he was not able to deflate the catheter and remove it. Hence he has put the needle inside the urinary bladder and punctured the balloon of the catheter and after that my catheter was removed.

On **-**-**** I was admitted again for Left Side Non-Functional Kidney, as per my discharge summary dtd:-**-**-**** it is clearly mentioned that “After doing complete paraesthesia check up patient under went laparoscopic left side simple nephrectomy + cystoscopy+right DJ Stent removal right relook URS done by Dr. Bijoy Abraham under the procedure went” in the above said summary there is a clear negligence of piece of catheter which was not seen by treating doctor.


On **/**/**** after * months of surgery I passed a piece of catheter in my urine along with blood . Firstly I was very much frightened that what I have passed through urine. After thorough investigation I came to know that it was tip of foley’s catheter which was lying adamant in my urinary bladder since **/**/****. By god grace I have not got any infection or injury because of that piece of rubber foleys’ catheter . I’m a lay man not very well versed with the medical parameters. But one thing I can understand that in my case it is very serious negligence from the hospital staff/treating doctor. Since it is the duty of the on duty staff as well as treating doctor to confirm whether the catheter which was removed came out in one piece or if they have got doubt that some part of the Foleys catheter may have remained inside the urinary bladder then they go for USG Abdomen /Pelvis to reconfirm the same. But in my case no USG Abdomen/Pelvis was done at that time.
Now When I contacted my treating surgeon on **/**/**** I had informed him the total story. He informed to me that such process is a normal one and it may happen often and he advised me to go for USG Abdomen saying that there may be some other parts of catheter may be present inside my urinary bladder. When the treating surgeon was not sure whether the Foleys catheter was completely removed then why he has not gone for the USG Abdomen/Pelvis at that time.
According to me the incidence which was happened to me is a crime on hospital part because due to such negligence patient have to pay heavily for the mistake which he has not done.
I request you to kindly do the complete investigation at the earliest so that this may not happen to any patient in the future. I request you to kindly revert back to me at the earliest.

I have paid my full bill of my first surgery Rs.*.** lacs and second surgery of Rs. *.** lacs through cash and card for * (Two) surgeries.
