United India Insurance Co Ltd - Health Insurance Canceled for no fault on mine

I have subscribed to Insurance through Andhra Bank account since **** (In West Anna Naga, Chennai Branch) to cover myself and my wife for Rs *L. The payment was regularly deducted from my Andhra Bank account.
In ****, I moved my account my account from Chennai Branch to Hyderabad Chandrapuri branch in LB Nagar near my home (IFSC Code ANDB*******) . The premium amount was Rs *****+, in ****. In **** (I received an SMS saying premium needs to be deposited by July *th, ****; however, they never mentioned how much will be the premium amount.
I had Rs ***** in account towards the premium on July *th, ****. But they never took my premium amount. In August ****, I called the Bank and enquired about why the Insurance Card was not issued.
Andhra Bank (now Union Bank) contacted the insurance Co in Hyderabad about my query. I was informed by the Bank that there was an issue about the entry of my name in insurance system and the insurance company asked the bank to clear it before the premium was taken from my account. The branch manager assured me that they cleared the issue with Insurance Company and the premium will be taken out from my account and asked me to keep at least Rs *****.
This was the first time I EVER Heard the premium amount was raised to Rs *****. However, in August I had deposited about Rs ****** into account.
I waited for another month to see if I get the insurance cards. I again contacted the bank and my branch manager wrote another letter asking for insurance company to issue my Insurance Cards. At that time, United India Insurance Company Hyderbad branch office took a position that the premium amount was increased to Rs ***** and in my account on July *th **** there was not enough money towards premium and hence they canceled my policy.
This position of the Insurance Company is gross negligence as they never is a GOOD EXAMPLE OF CLEAR DISCRIMINATION.
I am hoping that your Organization and/or Committee will do justice to my case.

At ** years age after paying for so many years to the insurance company, with only ONE claim during that period, they conveniently canceled my policy since I am a senior citizen.
