Upstox india - DIS slip rejection

I want to transfer my mutual funds from upstox account to my other demat account which is held with angel broking. In order to do so, I am required to send a duly filled and signed DIS slip to Upstox. Although to receive a dis slip, one needs to send a request form first so that DIS slip can be sent to customer's address. I have sent the form * times and they have denied sending me the dis slip stating that there is slight mismatch of my signature, which is utter rubbish. They just don't want to send me and know that after transfer I will close the account with them. So kindly help me out.
