*. I am writing this to lodge a formal complaint regarding damage to goods transported under consignment No CRN**********. The goods were picked up on ** May **** from Meena Ganga apartment Kestopur, and delivered on ** May **** to Gitanjali Apartment Barrackpore.
*. Upon receipt, I discovered the following damage:

(a) Almirah **" printed - ** - Rs *****.** - Totally damaged
(b) Dressing table wooden **/**- **- Rs ****.** - Totally damaged
(c) Mattress */* *" - ** - Rs ****.** - Torn from sides
(d) Bed Single - ** - Rs **** - Broken
(e) Bed double -** - Rs **** - Broken
Total cost of items are **,***.** Rs
*. I have attached the following evidence :-
(a) Photos of the damaged goods and packaging
(b) Copy of consignment Note
(c) Bill of the items
All the above documents are
*. I believe the damage was a result of negligence of the driver, or improper handling of goods during transit. As a result of this damage, I request full compensation for the value of the damaged goods.
