प्रार्थी वीरेन्द्र ठाकुर ने अपने वाहन टाटा टिआगो MP-**-CH-**** की पालिसी ऑनलाइन पालिसी बाज़ार एप्प के माध्यम से नेशनल इंश्योरेंस से करायी है | पालिसी रिन्यू करते समय RC Paper में कारऑनर मेरा नाम होने के बावजूद भी पालिसी बाज़ार एप्प की गलती के कारण नई पालिसी में पूर्व कार ऑनर का नाम बाय डिफाल्ट (एडिट नहीं हुआ )लेने के कारण टाटा कार शोरूम में क्लेम देने से मना कर दिया गया ,जबकि पालिसी में एड्रेस मेरे भोपाल स्थित आवास का है जो कि पालिसी रिन्यू करते समय जोड़ा गया था |
