LIC life insurence - Agent causing late fees for several times

My agent name is Sarika Sunil Borawake. However Sunil Borawade looks into these policies , I have never seen Sarika being a part of it , she was the one with whose confidence we took * policies. This is the second time this has happened he has taken blank cheques from my mom as she was travelling out of India. We asked him to pay the premium, in this he lost his phone and lost all contacts , we are trying to contact him.. his wife under whose name these policies are registered is not responsive.
When he contacted back he is giving us fine of **** last year he gave us fine of ****. He is giving us fine each year. We want him to be removed from our policy and want him to pay for his mistakes. My mom is a pensioner we took LIC for our benefit not to keep paying because of his mistakes
