Subject:- Decline of Claim without any specified reason for Claim ID ****** of Max Bupa Health Insurance Co.
Dear Sir,
As stated above, I got admitted in hospital on *th June ****, I have taken health insurance policy from Max Bupa Health Insurance Co. - Policy number-**************. My policy got issued on **-**-**** after all medical tests conducted by company. At that time, I have no any medical issues. You may also verify the test reports of Company which they conducted before issuing policy. This is a port policy also that means all the benefits of older policy should also continued by the new company.
There is no such kind of medical facts, which I hide from the company at the time of policy inception.
As of my medical history, very first I visited Dr.Rajeev Gupa (EHCC hospital) on **th May **** with complaints of uneasiness from last *-* days. After checking Dr diagnosed Hypertension and COPD. He suggested some investigations and prescribed some medicines to me. I revisited the doctor with my investigation reports on **th may and he advised me further treatment.
On *th June **** in evening hours I got admitted in hospital in emergency due to blood loss through nose. I undergo through medical treatment & got discharge on **th June ****. I applied for cashless claim from Max Bupa company and they raised a query about my first diagnosis. In accordance Doctor wrote a letter about my hypertension since *-* months & copd from * month, which is not correct. Hospital Staff asked some of my relative about this & Dr. wrote this, due to lack of knowledge of my relative for my medical conditions he casually told Staff for *-* months of hypertension but the fact is it is detected on **/*/** for the very first time.
For this I gave Dr’s prescription letter and investigation reports of **th may **** in original to company.
Even Though Dr. wrote for *-* months so what this policy is done after medical examination & also port case.
There is no such specific date of detection of hypertension given by the doctor, just *-* months casually written. On this ground how Max Bupa Company assume date of detection before policy inception date. I have given the relevant Dr’s prescription letter and investigation reports of **th may ****.
On the basis of above facts kindly check all things & also instruct Max Bupa company to reimburse my Claim.
