Dear Ma'am/Sir,
With the above-mentioned subject, I would like to submit my grievances against the Healthcare Services (https://www.healthcareservices.org.in/) which duped me in the name of OPD health reimbursement services.
The sequence of events is enlisted below for your kind perusal and necessary action.

Name of the employee: Simran and Prashant Dubey (HCS****)

Name of Supervisor: Pankaj Sharma (Employee ID HCS *****)
The sales executive Ms. Simran called on Jan **, ****, at **:** am and informed me about the OPD policy, and shared the policy details (which I wanted to cross-check and asked them I will let them know after two days).
January **, ****- Simran again called and enquired so I told her that I need more clarification and can call back in the evening.
To cross-check I called Pankaj Sharma who informed the IRDA code **.
January **, ****- **:** hrs again the Ms. Simran called and handed over the call to Mr. Prashant Dubey who told me that it is secure and the amount paid by Paytm link provided in my email. For the cashback, he asked me to buy the policy using a cashback coupon but instead of cashback, the amount Rs ***** was deducted twice from my account which is shared on his WhatsApp. The cashback was promised in **-** hrs but till this time no cashback has yet been received. January **-**, ****- I called Prashant Dubey and enquired but he is avoiding and answering in fraud answers.
Dubey told me that the policy document will be sent to my email which even after inking this email not yet received.
Fraud detected due to
*. When policy document not yet received.
*. Mr. Pankaj Sharma declined to have an IRDA code. And we have a legal team to handle the
issue.
*. Mr. Prashant Dubey asked to get scheduled body tests which are (Lal Pathological Laboratory) is
automatically generated after punching policy.
*. Ms. Simran not receiving call or Mr. Dubey hesitating to inform the exact reason for not allocating
policy.
*. Mr. Sharma not receiving calls.
*. I immediately informed my bank and blocked my credit card to avoid any further misuse.
I strongly feel this is fraud happened to me and the numbers are still active my appeal to the crime branch please help me in the refund of my money (Rs *****+*****= Rs *****.**).
This policy is ** days’ return policy and me or my family members have not even availed of any of the services falsely claimed by the company.
Please help so that other innocent people do not fall prey to such fraudsters.
