sir,
*. i have the honour to request that the claim settlement reply provided by your office against my above mentioned complaint (i.e. pmopg/e/****/******* dated ** dec **), is not solving my grievance as the details provided by your office is questionable and not up to date as per records i am having. kindly give attention to below mentioned points, reply of these will solve my problem:-

(a) in your office reply it was mentioned that from aug **** to feb ****, i have deposited premium **** rs. it is false information provided by your office. (copy of letter is attached for your reference).

(b) statement of premium history provided by your office is showing details from ** feb **** onwards to ** mar **** only. whereas policy no -********* d.o.c is ** aug ****. the premium details paid vide premium receipt no. ******* dated ** sep ****, premium receipt no-******* dated ** mar **** and premium receipt no- ******* dated ** dec **** are not included in the premium history statement. including these premiums i have paid ** premium installments. so the premium history statement provided by your office is not providing full information to solve my problem. (copy of premium history statement and above mentioned premium receipts copy is attached for your ready reference).

(c) sir, i want to draw your attention towards these missing premiums amount, while calculating my settlement claim these premiums amount is not considered, due to that my claim is reduced and i was not paid what i deserve against my policy. due to this mistake by your department, my policy status shown me less progress amount due to that i decided to surrender my lic policy.

(d) so, i request that my claim may please be re processed with including my missing premiums amount and difference of claim may please be paid to me and a clear readable copy of recalculated premium history may be provided to me with all benefits including all premiums. further, i also requests that defaulters may be punished due to them i am facing problem.
